Is Anki a must for Step 1?
As for whether anki is necessary, it is not BUT it all depends on how you learn. most people end up relying heavily on it. but i know people who got 250+ without anki. The most important part about anki is making sure you keep up with all of your reviews otherwise the system is worthless.
How many Zanki cards should I do a day?
Zanki can be rough long term, getting 200+ reviews in a single day. If you’re fast, you could get those done in 1-2 hours (I’m too ADHD to do that). To lower the number of reviews, I suggest a set of news from each subdeck on a different day (eg 60 from biochem on day 0 then 60 from cardio on day 1, etc.).
Is Zanki or Anking better?
Anking vs Zanki Anking is just the same as Zanki except that it comes with a better tagging system and a few additional decks.
Should I make my own Anki cards?
Yes, there’s definitely value in making your own deck. The two benefits I see are: 1) familiarizing yourself with the material when adding new cards; 2) and customizing the deck to suit your needs. I do both. My first deck is the best one I could find among the shared anki decks.

If you have a strong knowledge base and are performing well clinically, Zanki Step 2 is a great resource for you. If you need more guidance and background knowledge, WiWa might be a deck better suited to your needs.
